Any idea why the ground fill isn’t being connected to pads 2/3/4 here?

Not specifically but you seem to have some strange artifacts going on. Seeing your zone settings might help. Try lowering your clearance, raising the priority by one and hitting “b” to refill.
No change at all :(.
You have thermal spokes connection enabled which explains part of why it looks the way it does. The problem is that the three gnd pads near each other are in their respective anti pad clearance zone (the clearance created to get thermal spokes connections instead of full connections.)
Play with the thermal connection settings of your zone or select a different zone connection for these pads. (One option would be to select connection none and then make the thermal spoke manually by using a normal trace.)
