I’ve once designed a footprint with silkscreen (and other normally-on-top) layers on the bottom side. But that was specifically because the footprint was intended to be used for a shield (where the connecting board is intended to be mounted to the bottom side of the board). But I didn’t do anything different with the THT pads. I didn’t need to because I understood that THT pads will be on all manufactured layers.
This is a corner case implementation though, only done after intentional forethought.

What won’t work is a mirror of a TH part on a 2 layer board.
What is a really bad idea, is a mirror of a TH part on the same side as the copper.
